Novo Nordisk Field Safety Notice  -  NovoPen Echo and NovoPen 5

Novo Nordisk A/S has detected that the insulin cartridge holder used in a number of NovoPen Echo and NovoPen 5 batches may crack or break if exposed to certain chemicals in some household products, such as cleaning agent, sunscreen and food grease.  Certain batches are affected and patients are advised to the NovoNordisk Customer Care Line on 0845 600 5055 to check their device and/or to request a replacement.

New Patient Registrationregister_now

Please bring your NHS number, some photo ID and proof of your address (Utility bill, bank statement etc) to reception. If you do not have a medical card our receptionist can provide you with a registration form for you to complete. This form may also be used if you wish to register as an organ and/or blood donor.
NB:
You must supply: NHS number, Photo ID & Proof of address

You will be asked to complete a questionnaire about your medical history. In addition, you will need a new patient check with a practice nurse for the registration process to be completed. Please book an appointment with the receptionist.

If you are taking any regular medication you will need an appointment to see the doctor before a repeat prescription can be issued.  Please bring along all your medicines to this appointment.

We are required to provide a named and accountable GP for all patients.
Yours is the Registered and Usual GP automatically allocated when registering at Castle Healthcare Practice. If you require more information please ask at reception.  This may not necessarily be the GP that you choose to see.

If you move outside of our practice catchment area you will be sent a letter asking you to re-register at a practice closer to your new home.

Changes to Personal Details

Please notify the receptionist of any change of address or telephone number. Please also let us have your mobile telephone number.  If you give consent for SMS messaging, we can contact you to send appointment reminders and other messages.

We will never send confidential patient data by SMS message.
